VNA Home Health Hospice will be hosting flu vaccine clinics for both staff and students. The forms are included in the links below as well as available through the school nurses at each school.

October 1:

8:00-10:00 AM at Biddeford High School
10:15 AM-12:00 PM at John F. Kennedy Kindergarten Center
1:00-2:30 PM at Biddeford Intermediate School
October 2:
7:30-9:30 AM at Biddeford Primary School
10:00-12:00 PM at Biddeford Middle School
October 3:
9:00-11:00 AM at Dayton Consolidated School

 

Please read the Vaccine Information Sheets and complete the Health Screen & Permission Form and return to the school the day of the vaccine.  Please fill out insurance information correctly.  Although the vaccine is provided at no charge, there is a cost to administer the vaccine. All forms must be filled out completely. Children presenting with an incomplete form will not receive their vaccine. There will be no out of pocket cost.

All clinics will have an injectable vaccine. In addition, FluMist (nasal spray) may be available at some clinics (it cannot guarantee it for all clinics).  Unless there is a contraindication to one type of vaccine, the nurse at the clinic will determine which type of vaccine to use for your child.
